Our Day Habilitation services provide the participant with development and maintenance of skills related to activities of daily living, vocation, socialization, and participation in meaningful activities. ACI Day-Hab services support participants by providing access to meaningful activities to develop learning new skills, build positive social and interpersonal skills, attain independence, self-direction, and problem-solving. Safety practices in the home and community are built into our Day Habilitation curriculum and schedule.

Nursing Support Services

Nursing Support Services provides a registered nurse, licensed in the State of Maryland, to perform Nursing Consultation, Health Case Management, and Delegation services based on the participant’s assessed need. At a minimum, the registered nurse must perform an initial nursing assessment.

The purpose of this initial nursing assessment is to determine the participant’s medical needs, delegate nursing duties, and train/supervise direct care providers.

Personal Supports

Personal Supports offer skill-building and overnight assistance for individuals living in their own or family homes, helping them develop, strengthen, or maintain the abilities needed to enhance their independence. These services include the following:

  • Skill-building at home includes managing money and budgeting, completing homework, maintaining personal living spaces, practicing good tenancy, preparing meals, managing personal hygiene, handling household chores such as cleaning and laundry, and fostering daily independence.
  • Community integration and engagementskills development needed to be part of a family event or community at large. Community integration services support individuals in actively participating in daily life at home and within their communities. They may include the development of skills or providing supports that make it possible for participants and families to lead full, integrated lives (e.g., grocery shopping; banking; getting a haircut; using public transportation; attending school or social events; joining community organizations or clubs; any form of recreation or leisure activity; volunteering; and participating in organized worship or spiritual activities) and health management support for adults (e.g., learning to book medical appointments; exploring transportation resources; and building communication skills to express health needs, concerns, or conditions).

Supported Living

Supported Living services offer personalized assistance tailored to each participant, helping them live independently within their community. These services are guided by the individual’s Person-Centered Plan and are delivered in a way that reflects their unique needs, preferences, and goals. Supported Living services assist the participant in learning self-direction and problem-solving related to performing activities of daily living and instrumental activities of daily living required for the participant to live independently; and participate in community activities based on the participant’s interests and available resources. Supported Living services enable the participant to live in a home of his or her choice located where he or she wants to live and live with or without other participants or individuals of his or her choice. Supported Living services are provided in the participant’s home.

Day Habilitation

Day Habilitation services provide the participant with the following development and maintenance of skills related to activities of daily living, instrumental activities of daily living, and vocation and socialization, through application of formal and informal teaching methods and participation in meaningful activities.

  • Teaching methods based on recognized best practices are used through instruction.
  • Meaningful activities under this service will provide the participant with opportunities to develop skills related to learning new skills, building positive social skills and interpersonal skills, greater independence, and personal choice, including:

    • Learning skills for employment
    • Learning acceptable social skills
    • Learning effective communication
    • Learning self-direction and problem-solving
    • Engaging in safety practices
    • Performing household chores in a safe and effective manner and
    • Performing self-care.

Community Development Services (CDS)

Community Development Services help individuals build and maintain skills for active community participation by engaging in inclusive activities alongside people without disabilities. Community-based activities under this service provide the participant with access and support to engage in community-based activities for the development, acquisition, and maintenance of skills to increase the participant’s independence related to community integration with individuals without disabilities, such as:

  • Promoting growth and developing general skills and the social support necessary to gain, retain, or advance competitive integrated employment opportunities
  • Learning “appropriate” social skills that can promote community integration
  • Learning self-advocacy skills.

Community Development Services include the provision of direct support services that enable the participant to learn, develop, and maintain general skills related to participating in community activities. Transportation is included in this Waiver service.

Respite Care
